im using same exedy 400 with stock flywheel. also got a dorman TOB and still running after 15k miles. i cheaped out on the TOB the first time and it blew out at 500 miles.

I bought the slave I'm using with my Exedy Mach 500 clutch and flywheel package from RockAuto for something like $80.
