broken cam follower

I had a loud noise and a bad misfire in one hole. turned out to be a broken follower, my tech pulled the head and sent it to a machine shop. They said the guides were too tight on two valve stems. I have Livernois stage III heads but they have over 15k miles on them. Any suggestions?

I had one go out on me as well, only with the stock heads. Car had less than 40k on it. The only thing I can think of is the aftermarket cams having so much lift causes a premature failure but more than likely I'll just charge it to the shit happens account.
