    i had the same problem and just switched to the nylon lock nuts and havent had a problem since. i used to have to retighten them about once a month and eventually figured the constant movement loosened the nuts.

    did you use a scanner? most of the time a scan tool will give you information like : P0153: Oxygen Sensor Circuit Slow Response (Bank2, Sensor1) ,indicating the location of the sensor. but most likely that code is 1 of the 2 upstream sensors.

    yeah mine does the same. Just changed the leaky TOB a couple weeks ago, but for some reason noise goes away if i pump the clutch a couple times at a stoplight so i might have air in the system.

    Shouldn't I be throwing codes right now?

    You should have a low circuit voltage code unless the sensors were turned off in a tune. If you bought your car used the previous owner might have turned them off, at least thats what happened with me.
