fuel /air/spark/
one of those is off ,either too much , not enough or missing ..

its very odd whats going on, and the fact it "melted " the bar end off is nuts,
the bar dont look to have a "push" to it , so no foreign debris likely ..

it looks like it just over electrified and melted it clean off, ,,very clean

i sure hope its the same cylinder doing this, because then you have a very good starting point,,
check injector flow,, make sure the spark boot isnt cracked letting the spark arc along the way to the plug ,
doubt a valve is sticking and causing weird stuff to happen, ,again no POC seems to be going on here..
or a clone fake spark plug?
thats about all i got for ya,,, hope it helps somewhat

Melted electrode = lean condition. That cylinder is too lean, and it is misfiring. If it is only that one cylinder #5, it can only be a partially clogged injector, or a somewhat large vacuum leak at that jug. So no lean codes, only misfire?
