I have HR cams sitting waiting to have time to install them.
Here is the question: my car has 108k miles, would it be wise to freshen the timing when I do the cam swap?
I know increases to labor factor quite a bit, but I'd hate to put cams in and the timing let go!
I know a lot of you guys have much more knowledge and experience than I do and would appreciate you chiming in.

If you decide to go for the freshen-up, I recommend looking in to the FRPP 4.6L 3V Camshaft Drive Kit, part number M-6004-463V. It has everything in it that you need. Note this picture is of an early kit, they now come with the Cam Phaser Bolts as well.

Scott hit it on the head. If you do it that kit hs what you need!

To answer your question more directly, I would strongly suggest to at least replace the guides and tensioner!
