Possible FUBAR'd O2 sensor?

The only real problem I am having with the car is on throttle tip in, less than about 25% input, the car goes full lean, like in the 17's on my wideband, and bogs, doesn't pick up in RPM and speed until a half second or so later, then it goes into the 13's then the 12's. Instant WOT hit does the same thing, stays way lean for a half/split second and then goes to 11.5-11.7 or so.

No CEL's or anything... yet.

Ok well...

P0161- Oxygen O2 Sensor Circuit Low Voltage (Bank 2, Sensor 2)
P2272- O2 Sensor Signal Stuck Lean

Needs a new O2 sensor? Or could it be something in the tune?

Does it do it when it's cold? If so, that (maybe) bad sensor isn't the problem, since it doesn't look at the O2s until it goes into closed loop mode.

I've had cold start issues a lot, but only after it switches to closed loop mode. They've always been resolved in the tune by a) lengthening the delay time before it goes closed loop, and b) increasing the O2 heater wire duty cycle.
